CDR in MYSQL database

Hello.
I just finish install Asterisk 1.6.1.19 + Webmin 1.510 + ThirdlaneMultitanant 6.0.1.81.
Since i installed the standalone module i'm having problems using the mysql server to store the CDR data:
This is the ERROR message in console :

[May 10 12:33:19] ERROR[1874]: cdr_addon_mysql.c:195 mysql_log: Cannot connect to database server localhost: (1045) Access denied for user 'pbxmanager'@'localhost' (using password: YES)

I have the mysql server installed and running, but i'm not sure what database and table create to store the date. Is there any temaplte or script that someone could provide me to create the user/databes/table to make this work?
